ABOUT THE ROLE
We are looking for an experienced Care Coordinator who knows home health inside and out. You will own your cases, manage authorizations, coordinate with physicians and field staff, and keep everything documented and compliant — without being micromanaged.

WHAT YOU WILL DO
- Schedule skilled nursing and home health aide visits based on physician orders and individualized plans of care
- Contact physician offices to obtain signed orders, certifications, and plans of care — and follow up until they are received
- Submit and track insurance authorizations and pre-authorizations for all payers (Medicare, Medicaid, HMO, VA)
- Verify monthly patient insurance eligibility-
- Review EVV visits for accuracy and resolve discrepancies with field staff
- Serve as the primary point of contact for patients, families, and caregivers on scheduling and care coordination
- Monitor service utilization to ensure visits align with authorizations
- Maintain accurate documentation in our EMR system in accordance with AHCA, Medicare, and Medicaid guidelines
- Support general office operations and assist with compliance-related tasks as needed
WHAT WE ARE LOOKING FOR
Minimum 1 year of experience in home health coordination (home health strongly preferred over general medical office)
- Working knowledge of home health regulations, Medicare Conditions of Participation, and Medicaid waiver programs
- Experience with EMR systems and EVV platforms
- Familiarity with HMO authorization processes and insurance verification
- Highly organized with the ability to manage multiple priorities accurately
- Strong written and verbal communication skills
- Ability to work independently in a fast-paced environment
- Bilingual in English and Spanish — required
