Licensed Practical Nurse (LPN) – Private Duty

Locations

Allentown, PA

Key Responsibilities

  • Provide one-on-one skilled nursing care in the home and school setting
  • Deliver treatments and nursing services as directed by the care plan
  • Monitor patient conditions and report changes to the supervising Registered Nurse
  • Support implementation of individualized patient care plans
  • Educate patients and families regarding ongoing care needs and treatment plans
  • Assist with care coordination and communication among the care team
  • Maintain accurate and timely patient documentation
  • Ensure compliance with healthcare regulations and company policies

Required Qualifications

  • Current Licensed Practical Nurse (LPN) license in the state of Pennsylvania
  • CPR certification
  • Valid driver’s license
  • Minimum of 2 years of pediatric shift care nursing experience in any of the following settings: Diabetes care · Feeding assistance · G-tube · Medication Administration · Nebulizer · Non-ambulatory and Total care · Respiratory Assessment · Safety monitoring and Supervision · Seizures · Smart Vest · Straight Catheter · Suctioning · Tracheostomy (Trach) · Transfers · Tube feedings · Ventilators · VNS applications

Reporting Structure

This position reports to the supervising Registered Nurse

Work Environment

  • Primarily caring for pediatric patients in residential settings
  • Flexible scheduling opportunities, including full-time and part-time availability
  • Local travel required within the assigned service area
