Key Responsibilities
- Provide one-on-one skilled nursing care in the home and school setting
- Deliver treatments and nursing services as directed by the care plan
- Monitor patient conditions and report changes to the supervising Registered Nurse
- Support implementation of individualized patient care plans
- Educate patients and families regarding ongoing care needs and treatment plans
- Assist with care coordination and communication among the care team
- Maintain accurate and timely patient documentation
- Ensure compliance with healthcare regulations and company policies
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of Pennsylvania
- CPR certification
- Valid driver’s license
- Minimum of 2 years of pediatric shift care nursing experience in any of the following settings: Diabetes care · Feeding assistance · G-tube · Medication Administration · Nebulizer · Non-ambulatory and Total care · Respiratory Assessment · Safety monitoring and Supervision · Seizures · Smart Vest · Straight Catheter · Suctioning · Tracheostomy (Trach) · Transfers · Tube feedings · Ventilators · VNS applications

Reporting Structure
This position reports to the supervising Registered Nurse
Work Environment
- Primarily caring for pediatric patients in residential settings
- Flexible scheduling opportunities, including full-time and part-time availability
- Local travel required within the assigned service area
