Role & Responsibilities

• Provide specialized physical therapy services in a home health setting.
• Ensure patient care aligns with clinical standards, values, and mission.
• Maintain certification and licensure requirements for practicing in South Carolina.
• Collaborate with healthcare teams to develop and implement treatment plans.
• Document patient progress and adjust treatments as needed.

Qualifications & Skills

• Minimum of 2 years of Physical Therapist experience
• At least 1 year of recent home health experience
• Current Home Health certification in the state of practice
• Basic Life Support certification

Key Duties

• Assess patients' physical conditions and develop treatment plans
• Implement therapeutic interventions to improve patient mobility and function
• Maintain accurate documentation of patient care
• Adhere to all regulatory and safety standards
