Registered Nurse - Utilization and Care Transitions

The Utilization Management (UM) and Transitional Care Nurse Navigator/Liaison is responsible for building strong relationships with local hospitals, skilled nursing facilities (SNFs), and other care providers to ensure smooth transitions of care for PACE participants. The role focuses on improving care coordination, fostering collaboration, and ensuring compliance with federal and state regulations while enhancing the participant’s experience. Schedule: Monday-Friday, 8am-5pm. This position is onsite.
