Position: CASE MANAGER (MSW)
Responsibilities
The Case Manager is responsible for providing quality case management, admission support, utilization review activities and services to residents, their families and care providers. Serve as a member of treatment team supporting the organization’s treatment program and philosophy and assure the deliverance of quality treatment to residents and their families.

Oak Plains Academy
Oak Plains Academy is a comprehensive and highly structured 98-bed residential treatment program offering psychiatric residential treatment services to children and adolescents, struggling with emotional and behavioral issues. Residents receive an individualized treatment plan, geared to meet their personal needs. A professional team of psychiatrists, psychologists, social workers, therapists, nurses, and teachers serve hundreds of children and adolescents annually with the common goal of treating and healing trauma and building resiliency in the residents we serve.
Located on 20 acres of rolling hills in a rural setting, about 45 minutes from Nashville, Tennessee, Oak Plains Academy campus features a full-size gymnasium, dining hall, separate academic building, and four dormitories. Our wooded campus provides an excellent opportunity for outdoor recreational activities.
Qualifications
- Master’s Degree in a Behavioral Healthcare field required, Master’s degree in Social Work preferred
- LCSW License preferred
- Must possess knowledge of case management, discharge planning, age-specific growth and development, crisis and behavior management, individual and family therapy theory and practice, and behavioral health/substance use
- Prior experience working with a psychiatric behavioral health population between 1-3 years required, 3-5 years preferred
