Home Health Licensed Practical Nurse in North Little Rock, AR

Provide high-quality nursing care to patients in the comfort of their homes in this rewarding contract role as a Home Health Licensed Practical Nurse (LPN). Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Occasional flexibility to accommodate patient needs

Key Responsibilities:

• Conduct skilled nursing visits, assessing and implementing individualized care plans
• Administer medications, perform wound care, and manage clinical interventions as needed
• Monitor and report changes in patient conditions to the RN Case Manager or physician
• Educate patients and caregivers on health management, medication, and self-care
• Ensure thorough documentation of all care and communications using Home Care Home Base (HCHB)
• Adhere to infection control standards and uphold patient safety
• Collaborate with the interdisciplinary team to ensure comprehensive care
• Participate in case conferences and quality improvement activities

Requirements:

• Active, unrestricted LPN license in the state of practice
• At least 1 year of experience in home health, hospice, or similar roles
• Proficiency in Home Care Home Base (HCHB) charting system
• Current BLS certification
• Valid drivers license, reliable transportation, and current auto insurance
• Strong clinical judgment, time management, and communication skills
• Ability to work independently and prioritize a varied caseload

Preferred Qualifications:

• Experience with Medicare-certified home health agencies
• Strong understanding of home health compliance and documentation
• Demonstrated patient-centered, compassionate care approach
