Dallas Nephrology Associates has an immediate opening for an experienced nephrology social worker (minimal TX LBSW with 5 years of experience) to work with post transplant recipients, CKD patients and their families at our Dallas Transplant Institute located in Dallas, TX.

We are seeking an experienced Nephrology Social Worker to join our multidisciplinary team. Candidates with at least five (5) years of social work experience and direct experience working with transplant recipients are strongly preferred. A current Texas Licensed Baccalaureate Social Worker (LBSW) or higher is required.

This position provides comprehensive psychosocial support to post-kidney transplant recipients, patients with chronic kidney disease (CKD), and their families, with a primary focus on the post-transplant population.

Responsibilities include, but are not limited to, conducting ongoing psychosocial assessments to identify patients’ strengths, barriers, and needs related to maintaining a successful kidney transplant. The social worker will provide counseling, education, care coordination, and resource navigation to assist patients with issues such as returning to work, medication access, transplant pharmacy networks, changes in family roles, mental health concerns, crisis intervention, and advance care planning.

As an integral member of the post-transplant clinic, the social worker will collaborate closely with the transplant team to ensure patients have the psychosocial support and community resources necessary to maintain long-term transplant success. This role will also provide social work services to patients seen in the outpatient CKD clinics.

The successful candidate must be able to independently assess and triage complex psychosocial issues, effectively manage multiple priorities in a fast-paced clinical environment, and establish strong collaborative relationships with healthcare providers and community organizations.

Qualifications

  • Current Texas Licensed Baccalaureate Social Worker (LBSW) or higher (required)
  • Five (5) or more years of clinical social work experience, preferably in nephrology, transplantation, or another medical setting (strongly preferred)
  • Experience working with post transplant recipients or within a post-transplant program (strongly preferred)
  • Demonstrated ability to assess complex psychosocial needs, provide crisis intervention, and coordinate multidisciplinary care
  • Excellent communication, organizational, and care coordination skills

Pay: $75,000.00 - $80,000.00 per year
