Time Off & Holidays • We offer a combined paid time off (PTO) policy which incorporates state paid sick leave with company paid time off at an accrual rate of 0.0607 per hour worked, equivalent to 120 hours of PTO at 2,080 hours worked in a year. The PTO accrual rate increases after two years of service and again after five years of service

  • We offer 6 Paid Holidays (8 hours - full time, 6 hours - part-time per event, excluding on-call staff) and 1 Personal Holiday of Choice per year (excluding on-call staff)

Enrich the lives of older adults through community, choice, and continuing care in the Physical Therapist (PT) role. In this unique and rewarding role, you will help patients improve mobility, manage pain, and maintain independence-while also supporting comfort and quality of life for those in hospice. The Physical Therapist is solely responsible for evaluating and diagnosing a patient, preparing a treatment plan and therapeutic goals, including determining number and frequency of visits providing or supervising all care, and determining appropriate discharge of a patient. You'll work collaboratively with an interdisciplinary team to ensure that care is personalized, holistic, and aligned with each patient's values and goals. Find your sense of belonging at Wesley!

You will provide continuing care through these responsibilities

Essential functions of this position include the following. • Perform comprehensive assessments and develop individualized care plans based on physical, emotional, and functional needs

  • Assess and evaluate therapeutic, rehabilitative and/or functional status; participate in the development of the total plan of care
  • Assess for muscle strength, mobility gait, ROM-potential for rehab
  • Determine priority needs for therapy patients
  • Initiate Physical Therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with the patient, as well as instructing them as to the goals of the Physical Therapy program for the patient by participating in case conferences
  • Evaluate the home environment for safety
  • Deliver physical therapy services in patient homes, focused on restoring function, mobility, and quality of life
