Responsibilities:

• Evaluate patients to determine physical, cognitive, and emotional challenges affecting daily activities.
• Develop and implement individualized treatment plans tailored to patient goals.
• Improve patients' ability to perform daily living activities, including dressing, bathing, grooming, and fine motor tasks.
• Recommend adaptive equipment and environmental modifications to enhance patient safety and independence.
• Educate patients, families, and caregivers on safe techniques and home exercise programs.
• Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
• Complete documentation accurately and submit notes in a timely manner.

Qualifications:

Occupational Therapist (OTR)

• Active Arizona Occupational Therapy License

Certified Occupational Therapy Assistant (COTA)

• Active Arizona COTA License

Additionally:

• Current CPR/BLS Certification
• Valid Driver's License
• Reliable transportation
• Current Auto Insurance
• Excellent communication and organizational skills
• Home Health experience preferred (but not required)
• Minimum one year of clinical experience preferred
• Arizona Business License preferred for Independent Contractors

This is an Independent Contractor position, AZ Business License preferred.
