Position Overview
Above All Talent Solutions is collaborating with an innovative therapy provider that offers advanced rehabilitation programs to Senior Living communities. In this full-time Physical Therapist role, you may also provide support to sister facilities, working with adult and geriatric residents in a post-acute care setting. As a vital part of our interdisciplinary rehab team, you will empower residents to enhance their mobility, strength, balance, and pain management, ultimately improving their safety, independence, and quality of life.
Your Key Responsibilities
- Conduct evaluations, diagnose, and treat adult and geriatric residents facing mobility, strength, balance, gait, and pain challenges in a skilled nursing facility/long-term care environment.
- Collaborate with nursing, occupational therapy, speech-language pathology, and medical staff to create and execute tailored, goal-oriented treatment plans.
- Ensure timely and accurate documentation of evaluations, daily treatment notes, progress reports, and discharge plans in accordance with facility, payer, and regulatory requirements.
- Provide education to residents, families, and staff on therapeutic exercises, mobility aids, fall prevention strategies, and home safety measures.
- Engage in care conferences, quality improvement initiatives, and program development to foster strong clinical and operational outcomes.
Qualifications We Hope to See
- A current and unrestricted Physical Therapist license in the state of practice (or eligibility to obtain one before starting).
- Previous experience in skilled nursing, post-acute rehabilitation, or long-term care is strongly preferred.
- Strong communication, documentation, and collaboration skills with a resident-centered, outcomes-focused approach.
