Your Responsibilities:

  • Deliver psychosocial support to patients and families based on individualized care plans.
  • Conduct assessments to pinpoint social, emotional, financial, and environmental needs.
  • Utilize your professional judgment to help patients and families navigate the complexities of illness and loss.
  • Provide counseling, education, and facilitate referrals to community resources.
  • Work collaboratively with the hospice interdisciplinary team to address the comprehensive needs of those in your care.
  • Monitor changes in patient or family situations and adjust services as necessary.
  • Advocate for dignity, respect, and rights of every patient and family you serve.

Qualifications:

  • Bachelor's Degree in Social Work (BSW) from a CSWE-accredited institution.
  • Licensure as required by your state.
  • A minimum of 1 year of experience as a Social Worker in a healthcare or hospice environment.
  • Understanding of the psychosocial aspects of illness, grief, and end-of-life care.
  • Ability to provide compassionate support during emotionally challenging experiences.
  • Capacity to work independently while contributing significantly to a care team.

Preferred Experience:

  • 3 to 5 years of experience in hospice or healthcare social work.
  • Familiarity with terminal illness, grief responses, and crisis counseling.
  • Experience in working with diverse populations and family systems.
  • Advanced training or certifications in grief counseling or palliative care.
