Social Worker
The Social Worker is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of the patient’s plan of care from admission to post‑discharge. The Social Worker utilizes critical thinking skills, and the principles of case management to coordinate and implement a discharge plan that meets the patient’s needs and ensures a seamless, effective and efficient transition of care across the continuum.

Qualifications
- Current Licensure in Social Worker (LGSW/LCSW/LICSW) in the District of Columbia.
- Master’s degree in social work
- Attendance and Punctuality – Demonstrates the ability to arrive at the work site on time, limits the number of absences, provides adequate notice to supervisor for time off, takes lunch periods and breaks within prescribed times.
- Mandatory training, testing, and competencies completed on time – Competency Training; Services Excellence Training; Annual Physical.
- Three years’ experience in acute care or other related healthcare field is preferred.
- Case Management Certification preferred.
- Previous experience in case management and/or discharge planning is strongly preferred.
- Effective oral and written communication and interpersonal skills
- Knowledge of government and third-party reimbursement systems and contracts.
- Strong organizational and problem‑solving skills.
- Ability to maintain confidentiality.
- Ability to adapt to multiple and changing priorities.
