Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore or improve functional independence
• Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and caregivers on therapeutic techniques and adaptive strategies to support daily living activities

Required Qualifications

• Graduate of an accredited Occupational Therapy program
• Current and valid Occupational Therapist (OT) licensure in the state of Ohio
• Current NBCOT certification
• Prior experience working in a skilled nursing facility (SNF) setting preferred
• Current CPR/BLS certification

Preferred Experience / Skills

• Experience with geriatric patient populations in a post-acute or long-term care environment
• Familiarity with Medicare documentation and compliance requirements in a SNF setting
