Key Responsibilities

  • Conduct thorough evaluations that include reviewing the patient's medical history and performing clinical assessments to develop tailored treatment plans.
  • Clearly communicate evaluation results and treatment strategies to patients and their families, involving them in goal-setting discussions.
  • Implement evidence-based treatment protocols, adjusting strategies based on the patient’s ongoing progress and health status.
  • Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive patient care, documenting any changes in condition that necessitate revisions to the treatment plan.
  • Prepare detailed discharge plans and follow-up care strategies, documenting all patient interactions accurately and in a timely manner.
  • Meet productivity expectations through effective time management, maintain an organized schedule, and actively participate in marketing and outreach efforts.
  • Work towards departmental objectives while adhering to up-to-date health regulations and accreditation standards.
  • Engage in continual professional development by pursuing ongoing education and training opportunities to enhance your skills and knowledge.

Patient Population / Client Focus

The patients you will work with come from diverse backgrounds and present a variety of conditions requiring physical therapy intervention. Your expertise will be crucial in helping them recover from surgeries, manage chronic conditions, or regain strength after injuries. Fostering strong relationships with patients and their families will be at the core of your work. As a home care therapist, you will also assess the living conditions of patients to tailor treatments that consider their day-to-day environments and challenges.

Qualifications

  • A Bachelor's degree in Physical Therapy from an accredited institution.
  • A valid license to practice as a Physical Therapist in Florida is required.
  • Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is mandatory.
  • While previous experience is not required, a passion for helping others and a desire to learn are highly valued.

Preferred Experience

While not mandatory, candidates with experience in home care settings or those familiar with outpatient therapy protocols will have an advantage. A background in utilizing electronic health records (EHR) systems is also beneficial, as documentation is a key aspect of ensuring continuity of care.
