What You’ll Do:

  • Resident & Family Interviews: Conduct structured, face-to-face interviews to capture daily preferences and mood logs. Partner with families and floor staff to verify data for cognitively impaired residents.
  • Cognitive & Mood Assessments: Administer standardized screening tools, including the BIMS (Brief Interview for Mental Status) and PHQ-9.
  • Chart Auditing & Behavior Tracking: Review medical records, nursing notes, and therapy logs. Document and code behavior patterns, mood shifts, and exit-seeking behaviors.
  • Own the MDS Process: Accurately log, coordinate, and execute the Social Services sections of the MDS on time.

What You Bring to the Table

  • Education: Bachelor’s degree in Social Work (BSW) or a related human services field.
  • Experience: 1–2 years in a skilled nursing facility with exposure to the RAI process.
  • Technical Skills: Experience with Electronic Health Records (EHR) software like PointClickCare.
  • The Vibe: Acute attention to detail, strong clinical interviewing skills, and a patient heart for the geriatric population.

Job Type: Full-time

Pay: From $35.00 per hour
