What You'll Be DoingDeliver Exceptional Therapy

• Complete in-home assessments focused on mobility, balance, and strength
• Develop and implement individualized treatment plans
• Provide therapeutic interventions and exercise progression
• Recommend adaptive equipment and safety modifications
• Educate patients and caregivers to maximize outcomes

Collaborate Across The Continuum

• Partner with nurses, aides, physicians, and interdisciplinary teams
• Communicate progress and adjust treatment plans as needed
• Document visits and monitor patient outcomes
• Help patients remain strong, safe, and independent

Required Qualifications

• Active Minnesota Physical Therapist license
• Valid driver's license and reliable transportation
• Strong communication and clinical reasoning skills
• Ability to work independently and manage your schedule effectively

Preferred Qualifications

• Home health experience
• Outpatient, inpatient, SNF, or rehabilitation experience
• Passion for relationship-based, patient-centered care
