Hospice Registered Nurse (RN) – Job Description
Position Summary
The Hospice RN is responsible for coordinating and providing high‑quality, compassionate nursing care to terminally ill patients and their families. The RN oversees the patient’s plan of care, ensures symptom management, and collaborates with the interdisciplinary team to support comfort, dignity, and quality of life.
Key Responsibilities
- Conduct comprehensive initial and ongoing nursing assessments.
- Develop, implement, and revise individualized hospice plans of care.
- Provide skilled nursing care including pain and symptom management.
- Educate patients and families on disease progression, medications, and comfort measures.
- Coordinate care with physicians, social workers, chaplains, CNAs, and other team members.
- Perform regular visits per hospice regulations and patient needs.
- Document all care accurately and timely in the EMR.
- Participate in on‑call rotation as assigned.
- Ensure compliance with Medicare, state, and agency hospice standards.
- Support families through end‑of‑life processes and bereavement transition.
Qualifications
- Current RN license in the state of practice.
- Minimum 1 year of nursing experience (hospice or home health preferred).
- Strong clinical assessment and critical‑thinking skills.
- Valid driver’s license and reliable transportation.
- Compassionate communication and ability to support families during crisis.
Hospice Licensed Vocational Nurse (LVN) – Job Description
Position Summary
The Hospice LVN provides direct patient care under the supervision of the RN Case Manager. The LVN supports comfort-focused care, assists with symptom management, and ensures patients receive compassionate, timely nursing interventions.
Key Responsibilities
- Provide direct nursing care according to the established plan of care.
- Monitor and report changes in patient condition to the supervising RN.
- Perform nursing tasks such as wound care, medication administration, and vital signs.
- Reinforce patient and family education provided by the RN.
- Document all care and observations accurately and promptly.
- Participate in routine and PRN visits as assigned.
- Communicate effectively with the interdisciplinary team.
- Maintain compliance with hospice policies and regulatory standards.

The Hospice CHHA provides compassionate, hands‑on personal care to terminally ill patients under the supervision of the RN Case Manager. The CHHA supports comfort‑focused care, assists with activities of daily living, and helps maintain the patient’s dignity, safety, and quality of life throughout the end‑of‑life journey.
Key Responsibilities
- Provide personal care including bathing, grooming, oral care, toileting, and dressing.
- Assist patients with mobility, transfers, and safe ambulation.
- Support comfort measures as directed by the RN (positioning, skin care, fall prevention).
- Observe and report changes in patient condition, behavior, or environment to the supervising RN.
- Offer emotional support and companionship to patients and families.
- Maintain a clean, safe environment by assisting with light housekeeping related to patient care.
- Reinforce education provided by the RN regarding comfort, safety, and care routines.
- Document all care and observations accurately and promptly in the EMR.
- Participate in routine and PRN visits as assigned.
