Job Summary
Provide physical therapy services to patients in their homes, focusing on functional independence, mobility, strength, fall prevention and long‑term recovery. Manage case planning, documentation, and education for patients and caregivers while collaborating with interdisciplinary team members.
Key Activities
- Develop a patient‑ and family‑focused plan of care with standardized, goal‑oriented interventions.
- Maintain accurate and timely daily documentation.
- Apply standard treatment procedures.
- Provide teaching to patients and caregivers.
Schedule & Workload
- Full‑time (40 hours) – Monday through Friday.
- Average of 5–6 visits per day.
- Dedicated time for documentation, chart review, visit preparation, and care coordination.
Coverage Area
Primary coverage in Roxbury; also serves Dorchester, Jamaica Plain, and Mission Hill.

Qualifications
- One year of experience in acute care, rehabilitation, or home care as a Physical Therapist.
- Registered with the Board of Registration in Massachusetts.
- Excellent verbal, written, and electronic communication skills.
- Computer proficiency and ability to learn new software.
- Ability to prioritize work and adapt to changing situations.
- Travel required within Mass General Brigham Healthcare at Home’s geographic area.
